The Bridge Model featured in the NTOCC Fall 2013 Impact Newsletter

The National Transitions of Care Coalition (NTOCC) featured the Bridge Model in its Fall 2013 Impact Newsletter, highlighting recent successes of the Bridge Model in reducing hospital readmissions and in increasing primary care provider follow-up. The article describes how the Bridge Care Coordinator (BCC) works with the client, caregiver, and the entire care team to ensure a safe transition:

Throughout [the intervention], the BCC acts as a support system, advocate and patient activator… While Bridge approaches transitions from a psychosocial perspective, it is deeply rooted in team-based care.

The article was authored by ITCC member Robyn Golden, of Rush Health and Aging. Rush is a member of the NTOCC Advisors Council.
